At Lotus Medics we understand that “normal” can be different for different women, but everyone understands safety. And when you are bringing your precious bundle of joy into this world, safety is key and cannot be overemphasised enough.
A physiological birth which is natural does not require a midwife or a doctor. But pregnancy and labour is a dynamic process and there can be situations that arise that may deviate from the natural course or physiology. Whilst the journey, whether it be low risk or high risk pregnancy or a birth which can be a spontaneous vaginal delivery, or a forceps, or vacuum or a Caesarean section can be different for different women, what we all must focus on is the outcome – a healthy baby, a healthy mother and a happy family.
At Lotus Medics we understand these different dynamic processes of pregnancy and birth. Whilst we strive to maintain the physiology at all times, we are prepared for the deviations in the pregnant state and labour and are prepared not only to recognise them but to help you adjust with them so that our goal of a safe outcome is ensured.

Pre-Pregnancy Counselling
Prepares a woman for her upcoming pregnancy by optimising her medical condition, medications and preparing a plan for her pregnancy based on her previous deliveries and health concerns and her wishes. This also includes investigating couples who have had recurrent miscarraiges and previous traumatic deliveries and delving into factors that are correctable so that subsequent pregnancy outcomes are favourable.
Antenatal Care
Incoporates a customised care for each woman, focussing on wellbeing in pregnancy and a plan for safe delivery
- First trimester - Initial appointment with dating scans, screening for chromosomal anomalies including NIPT(Non-Invasive Prenatal testing), antenatal booking appointment, and blood tests, etc.
- Second trimester - ultrasound scans for cervical length, morphology scans for detecting structural defects, tests for screening for diabetes in pregnancy.
- Third trimester - Preparing a comprehensive pregnancy and delivery plan based on the risk factors, events in pregnancy so far and taking into account the wishes of the pregnant woman and the principles of informed consent.
- High risk pregnancy situations - Pregnancy is an ever-evolving dynamic situation and whilst one would like to think that all is going smoothly, sometimes things do not go according to plan and high risk situations arise. Dr Kotasthane has 21 years of experience in dealing with all high risk situations that may arise, such as: diabetes in pregnancy, hypertensive disorders like pre-eclampsia, renal disease, heart disease, autoimmune disease, endocrine disorders, placenta previa, multiple pregnancy, multiple surgeries including multiple previous Caesarean sections, high BMI and previous gastric banding surgery, previous traumatic deliveries and mental health situations, and many more.
- Multidisciplinary input - involving Midwives, physicians, endocrinologists, physiotherapists, dieticians, diabetic educators, psychologists, etc.
Delivery
“Towards Safe Delivery” is the motto of Lotus Medics. This involves informed choice for patients and their birth partners. A birth plan is made for delivery at 28 weeks visit and finalised at 36-37 weeks after taking into account the risk factors and patient wishes involving their birth partner at each step. Dr Kotasthane is proficient in all managing all types of high-risk delivery situations if they arise, including spontaneous vaginal deliveries, forceps, ventouse, caesarean sections, shoulder dystocia, etc. Dr Kotasthane encourages natural onset of labour and uses induction only in cases where delivery needs to be expedited for maternal or fetal concerns.
Multidisciplinary input – midwives, lactation consultants, physiotherapists, Anaesthetists and pain specialists, doulas, psychologists, etc.
Postnatal
Dr Kotasthane’s job is not done after he places your loved one in your arms! A full debrief of the delivery process is carried out so that women and their birth partners have the opportunity to answer questions and are happy and satisfied with the care received. Postnatal appointments offer opportunities to discuss the delivery, lactation, choices of contraception, change in medications, and make a plan for any future pregnancies to close the loop, so that women are supported at every step.
